This Parental Consent ("Consent") allows a parent or legal guardian to authorize life coaching for their child (the "Minor") with Encouraging You – Faith-Based Life Coaching, LLC ("Coach"). Please read it in full before signing.
1. Consent to Participate
I give my permission for my child to take part in life coaching sessions with Terri R-Martinez. I confirm that I am the Minor's parent or legal guardian and have the authority to give this consent.
2. Nature of Coaching
Life coaching offers encouragement, guidance, and accountability to help my child build confidence, gain clarity, and grow. It is not therapy, counseling, or medical or mental-health treatment, and it is not a substitute for care from a licensed professional.
3. Sessions & Parent Presence
- A parent or legal guardian must be present for each of the Minor's coaching sessions, whether in person, by phone, or by video call.
- Faith-based guidance is available and is included only if the family would like it.
4. Confidentiality & Safety
What my child shares is treated with care and kept confidential to build trust. However, if the Coach believes my child may be at risk of harm to themselves or others, or where disclosure is required by law, the Coach may share what is necessary with me or the appropriate authorities.
5. Financial Responsibility
As the parent or guardian, I am responsible for session fees under the coaching agreement (Cash, Zelle, or Stripe), including the 24-hour cancellation policy.
6. Acknowledgment
By signing below, I confirm that I have read and understood this Consent and voluntarily agree to my child's participation in coaching.
